To obtain a nutrition evaluation, contact us directly or ask your veterinarian for a referral. We work closely with specialists to provide in-depth support for pets with underlying health conditions.
Patient Information Needed for Nutrition Services Before Appointment
- A detailed list of all foods fed (brand, type, quantity) including commercial diets, treats, and table scraps. To facilitate this process, we recommend that you use a kitchen scale to weigh all food items fed each day and complete a diet history form that we will send you BEFORE the appointment. It helps to send pictures of the labels and ingredients so that we can identify the foods that you are feeding.
- A detailed list of all supplements and medications being given (including dose and frequency and photos of the labels of any supplements)
- A copy of recent bloodwork (complete blood count, chemistry, thyroid level), collected within the last six months (this may be requested or completed at the time of appointment)
- A complete copy of patient’s medical records
- A detailed recipe for any home-prepared diets (including how the food is prepared and exactly what ingredients are used (e.g. skin or no skin, drained or not drained, salt added, ideally with cooked or uncooked weights of ingredient, etc.)
