Medical Record Request Form

Disclaimers:

  • If you are requesting as a non-listed owner, non-listed clinic/rdvm, or non-insurance company, your request may need to be verified or authorized with the owner of the account.
  • If the destination is to a clinic/referring veterinarian that is not listed on the account, we will have to reach out to the owner for authorization. Please be advised this can take up to multiple business days. If you are needing records expedited, we recommend having the owner reach out to us directly to ensure the clinic/referring veterinarian is listed. This may also include any unlisted email addresses, as we are only able to send records to email addresses listed on the account.
  • If the destination is to a third-party/non-veterinarian facility, we may not be able to provide records to them. This can typically apply to breeders, boarders, rehab clinics, studies, trainers, shelters, rescues, unlisted owners, etc.
  • If the destination is to a legal entity, please contact our medical records department directly at (352) 294-4556.
  • Our Medical Records Department is open Monday through Friday from 8:00 a.m. – 5:00 p.m. Requests received after hours will not be received until the next business day.
