This request form is for our Small Animal Hospital (UF SAH) and Pet Emergency Treatment Services (UF PETS) locations only. If you are needing records from our Large Animal Hospital (UF LAH) or World Equestrian Center (UF WEC) locations, please reach out to their teams directly.
If you are not currently listed on the patient’s account as an owner or clinic, your request may need to be verified with the account owner, which can take several business days. If you are needing records expedited, we recommend having the account owner reach out to us directly to ensure that you are listed and given access to the records and medical information. Note: We are only able to send records to authorized email addresses listed on the account.
If the destination is to a third-party/non-veterinarian facility, we may not be able to provide records to them directly. This typically includes breeders, boarders, non-DVM rehab clinics, external studies, animal trainers, rescues and shelters, unlisted owners, etc.
If the destination is to a legal entity, Animal Services or Law Enforcement (for patients not under their ownership), please contact our medical records department directly at (352) 294-4556.
Our Medical Records Department is open Monday through Friday, from 8:00 am – 5:00 pm, and is closed on most national holidays. Requests received after hours will not be received until the next business day.
